Jack was born on September 21, 1935 and passed away on Thursday, April 20, 2017.

Jack was a resident of Indiana at the time of passing.

Raised in Monticello, Indiana, he was a graduate of Monticello High School in 1953.

He then served during the Korean War era with the United States Army.

He was a member of Wildcat Conservation Club of Mulberry, Masonic Lodge 103 F. & A.M. of Dayton, and the Rotary Club of Lebanon. Visitation is planned for Sunday, April 23, 2017, from 2:00 to 6:00 p.m., at Myers Mortuary, 1502 North Lebanon Street, in Lebanon, Indiana, with Masonic Rites beginning at 5:30 p.m. Funeral Services will be held on Monday, April 24, 2017, at 10:00 a.m. at the funeral home in Lebanon, with Rev. Anthony Stone, officiating. Interment with military rites will follow services at IOOF Riverview Cemetery in Monticello, Indiana.
